Profile: Assessment, Support Planning & Risk Management

This one day course is designed to enable staff to reflect upon the skills required to work successfully with service users in terms of assessment and support planning. This interactive day is designed to promote dialogue between participants and self-reflection and will cover the principles of good practice around person-centred support and assessment.

  • Key skills of support planning and risk assessment 

  • Best practice for daily notes, recording changes, and updating documents 

  • Recording outcomes using SMART goals
